PROXY FOR ANNUAL MEETING OF CONGREGATION TEMPLE SINAI FOUNDATION
The undersigned hereby appoints Saundra Levy as my proxy to attend the Annual Meeting of
the Congregation Temple Sinai Foundation to be held on May 16, 2023 or any continuance or
adjournment thereof with full power to vote and act for me and in my name, place, and stead
in the same manner, to the same extent, and with the same effect that I might were I
personally present thereat, giving to such proxies full power of substitution.
The undersigned instructs such proxies to vote as specified below with the understanding that,
unless a contrary choice is specified, this proxy will be voted “FOR” the various proposals
thereinafter listed.
I further authorize my proxies to vote on all such business that may come before the meeting in the manner they see fit as fully as if I were present and voting in person.
